2017-2018 PTSA Membership form
2016-2017 Budget– On July 8, 2016, a General Membership Meeting was held, and this is the budget that was approved. This will be the budget plan for the 2016-17 school year. If there are *major* changes requested, a Special General Membership Meeting will be held to get approval from the General Membership.
Cash Box Request – Holding a PTSA Event that will require making change . . .use this form to request a cash box.
Request for Payment – Use this form to request reimbursement for a purchase made on the behalf of the PTSA (prior approval suggested).
Deposit Form – This form should be used after each event that has generated PTSA funds that need to be deposited into the PTSA bank account.
Teacher Mini Grant Request 2016-17– For use by the teachers of WLMS to request funding for classroom curriculum projects. Forms should be submitted to Mrs. Adkins for approval and then will be forwarded to the PTSA for final approval and processing.
Fall 2015 Spirit Wear – Preorders due by September 18th. Limited quantities available in the school store after September 30th.
Pink Out T-shirt Order Form – Specially designed shirts for our Pink Out Games in October. Orders due by September 14th.
